As a Telehealth Physician, you will play a crucial role in providing high-quality remote medical care to patients living with diabetes, obesity, high blood pressure, and hypertension. You will collaborate closely with our multidisciplinary team to deliver personalized care plans, monitor patient progress, and empower individuals to manage their health effectively. </p><p style="padding: 0; margin: 15px 0 15px 0"> Perform comprehensive medical assessments, review patient medical histories, and order appropriate diagnostic tests as needed. </p></li><li style="margin-bottom: 5px"><p style="padding: 0; margin: 15px 0 15px 0"> Develop individualized treatment plans tailored to each patient's unique needs, incorporating evidence-based guidelines and best practices. </p></li><li style="margin-bottom: 5px"><p style="padding: 0; margin: 15px 0 15px 0"> Prescribe medications, lifestyle modifications, and other therapeutic interventions to optimize patient health outcomes. </p></li><li style="margin-bottom: 5px"><p style="padding: 0; margin: 15px 0 15px 0"> Educate patients about their conditions, treatment options, medication adherence, and preventive measures to minimize disease progression and complications. </p></li><li style="margin-bottom: 5px"><p style="padding: 0;
